Self Identification
It is the student’s
responsibility to notify Disability Services for Students of an
impairment/condition which may require accommodations. A student who wishes to
seek services must submit the DSS Application and Release of Information
forms along with appropriate documentation of
his/her impairment/condition to the Coordinator of DSS. This information can
be delivered to DSS by mail, fax, or in person. Once DSS receives this
information, the student will need to arrange an appointment to meet with the
Coordinator of DSS to determine eligibility and appropriate accommodations.
Determining Eligibility
A student is eligible to
receive services from DSS if he/she has been admitted to Trinity University
and has a physical and/or mental impairment that substantially limits a major
life activity. Determination of eligibility for services and appropriate
accommodations is made on an individualized, case-by-case basis.
Confidentiality
Files
of students with disabilities are considered educational student records and,
therefore, are protected by the Family Educational Rights and Privacy Act
(FERPA). In accordance with this law, Disability Services for Students
maintains confidentiality of the documentation materials and other
information gathered in the process of determining eligibility for services.
DSS will not release any documentation without a student’s informed
written consent, except as required by law or as deemed necessary to prevent
harm to the student and/or others.
On a legitimate,
educational need-to-know basis, DSS may discuss the impact or impairments
caused by the disability and the corresponding approved accommodations with
appropriate individuals on campus. Circumstances may include academic
accommodations, housing arrangements, instructional strategies, and resources
or other circumstances specific to the individual.
If the student's
disability involves a condition with implications for his/her physical health
or a condition which requires regular medication, the student will be encouraged
to notify Health Services
or sign a release for DSS to convey pertinent information.
