This document is also available as a Microsoft Excel spreadsheet, which may be saved as a document template.  To do so, open the file, and from the File menu select "Save As" and choose a file name for the form (for example "Grad fin asst"), changing the file type to "Document template." Then to fill out the form, choose "New" from the File menu (NOT the New document icon), pull up the "Spreadsheet Solutions" tab, and you will see the document template with the name you have chosen. Double-click on the icon, and save the form as a normal document with a name indicating the semester (for example "Grad fin asst fall 1999"). Using the form as a template can be more efficient than loading it into a manual typewriter.  But one caveat about using the form in Excel: It is best to write the page numbers in manually; otherwise the same page number will print on every page.

Any alternative format is also acceptable so long as it meets the requirements of the Office of Academic Affairs, and provides the same information.
