Health Services
are available to any currently registered student who has a complete
Health Record form on file. This includes a completed physical exam
form as well as information regarding immunizations and health insurance
coverage. All full time students must submit this form to the Health
Services before their initial registration at Trinity University. This
Health Record includes the consent for treatment, signed by the student
or parent /guardian if the student is a minor. The Health Record form
is mailed to new students after they have paid their admission deposit.
We request your adherence to our requirement, supported by the Texas
Department of Health, that student present proof of immunizations for
diphtheria, tetanus, polio, rubeola/ measles, mumps and rubella.
