be an active consumer in determining treatment. All services are
confidential.

* Health Services maintains
a current referral list of local physicians. When a student sees an off
campus physician, the student is responsible for securing transportation
and paying the costs incurred.
Charges
and Fees
Trinity University does not assess a student
health fee. There is no charge for
the nursing evaluation of a minor illness or injury, first aid treatment,
oral over the counter medications, health counseling or medical
referrals. Charges for
vaccinations, other injections, laboratory testing, extensive dressings
and orthopedic supplies will be at or near cost. A regularly scheduled clinic
appointment with the University Physician is $15. Students who miss appointments without
at least 2 hours notice will be billed $15.
Fees
associated with care in Health Services may be paid with check, charged
to the student’s Tiger Card or billed to the student’s
university account.
Health
Services is not a participant of any HMO, PPO or private insurance plan.
Therefore, we are unable to generate an insurance claim form to file with
your private insurance. We can provide you with an itemized bill,
but it is your responsibility to seek reimbursement from your insurance
carrier. There is no guarantee that charges incurred in Health
Services will be reimbursed to you by your insurance carrier.
If
the physician orders prescription medication, the student is responsible
for payment. If the student has a prescription drug card, they should be
familiar with the pharmacies that honor their plan and notify Health
Services at the time the prescription is being ordered. The student
is responsible for presenting their insurance information to the pharmacy
when they pick up their medications.
Patients’
Responsibilities
1.
To provide accurate information about
your health, including past illnesses, hospital visits, use of medicines
(prescription and over the counter), and use of any other type of substances
including vitamins, herbal supplements, alcohol and street drugs.
2. To
ask questions when you do not understand information or instructions.
3. To
inform the Health Services’ staff if you believe you cannot follow
through with the plan of care or treatment of your condition.
4. To
recognize the effects of your chosen lifestyle on your personal health.
Your health depends not just on your medical care and treatment, but also
on the decisions you make in your daily life.
