A number of processes related to research at Trinity require internal approval or sign-off. This page is a clearinghouse for these documents.
The Research Programs Coordinator is moving towards creating web-based versions of these forms, when appropriate.
Forms required before grant proposal submission
In keeping with responsibilities outlined in the university handbook, faculty and staff must obtain institutional approval prior to submitting any proposals or applications for external funding. Prior to submitting any proposal for external funds, please complete an Internal Approval Form (IAF). The IAF must be signed by you, your department chair, the Manager of Grants Accounting and the Associate Vice President for Academic Affairs: Budgets & Research. Proposals submitted to private foundations may require prior approval from the Office of the President.
Individuals submitting proposals that promise any sort of institutional commitment, must also submit a cost-share request in writing to Diane Smith at least 3 weeks before th proposal deadline..
Forms for grant-holders
Faculty who have active grants must complete an Annual Disclosure Form addressing possible conflicts of interest once a year. This requirement is outlined in Section Seven of the Faculty and Contract Staff Handbook.
Faculty wishing to hire an employee with funds from a grant must complete a Request to Appoint an Externally-Funded Employee
form. If you are not sure whether the person you wish to hire should be classified as an employee or a contractor, please contact Andrea Spanley in Payroll.
Forms for faculty supervising student researchers
Faculty who wish to hire students to work on research projects during the summer must complete and have the students sign a Stipend Disbursement Form.
If your student is not a U.S. citizen or permanent resident, please make sure he or she submits a signed Tax Worksheet along with the completed stipend form. Note that international students may not be paid from certain federal grants. International students (F-1 student visa) are eligible for on-campus employment for up to 20 hours per week during fall and spring semesters and up to 40 hours per week during official school breaks, i.e. spring break, winter break and summer semester).
If you wish for your students(s) to earn course credit for their research work with you over the summer, you must submit their names, along with a course number, to Rhonda Lewinson in the Registrar's Office by Friday, April 24, 2009. Students supported by certain grants are eligible for one free tuition credit for summer work. Ineligible students will have to pay tuition to receive research credit.
If student research will take them off campus, faculty mentors must familiarize themselves with Trinity's fieldtrip policy. At least one month before any off-campus trip, the mentor must provide Sheryl Tynes with the names of the students, faculty member, department, course number (when applicable), departure and return dates, and destination. Mentors should request confirmation of insurance coverage for the participants from Diane Smith. Students should sign an Informed Consent and Assumption of Risk Form.
Forms for student researchers
Students who have a confirmed research contract and who wish to request on-campus housing, must submit a Housing Reservation form by Monday, April 13, 2009.
Other Forms
Faculty and students engaging in research that involves human subjects must obtain approval from The Institutional Review Board (IRB). Please email the approval request along with supporting documnents to IRB@trinity.edu
