I. Instructions for Faculty.
1. Complete and print the
Internal Approval
Form (IAF). (Here in
Word.)
2. Have the Chair of your department read your proposal and sign the IAF.
3. Bring proposal, IAF and, if necessary,
Cost-Share Document
(Here in
Word)
to Mary Jump
in the Business Office at least
5 business days
before your deadline.
4. The proposal, IAF and Cost-Share Document will be forwarded to
Diane Smith. The Office of Academic Affairs will notify you
when your proposal is approved.
4. If your grant is
funded you will be required to complete an
Annual Disclosure Form addressing possible conflicts of interest.
(If you answer YES to any of the Questions Related to Business Interests
(4-9), you must complete
Part
B of this form.) The Annual Disclosure Form is to be signed and
submitted to the AVPAA:BR each June for the term of the grant. See
Part III. B of the
Conflict of Interest Policy in the Faculty Handbook.
