Good Afternoon,    

 

All students registering for fall classes, undergrad or graduate, are required to provide proof of Personal Health Insurance, by completing the on line waiver or enrolling in the Student Health Insurance Plan.  Students may submit this information on line beginning March 15, 2009.  The forms can be accessed through the Health Services web site at www.trinity.edu/healthservices. Students enrolling in the Student Health Insurance Plan before July 1 will have the premium of $724 for 12 months added to the student account for summer billing.  Details about this plan are available at www.ahpcare.com/trinity.    

 

The deadline for submitting proof of Health Insurance for 2009-10 academic year is March 23, 2009.  Registration holds will be placed for students who have not submitted proof of insurance by March 23.  The hold will be removed when the student provides proof of insurance or enrolls in the Student Health Insurance Plan.

 

Students who have failed to provide proof of insurance by July 1, 2009 will be enrolled in the Student Health Insurance Plan and the premium will be added to the bill for fall semester.   If you have been enrolled in this plan and you do not want it you must send a written request for cancellation to Health Services before August 10, 2009.  Proof of personal insurance must accompany this request.  After August 10 refunds cannot be made, except as specified in the policy.
