Service Forms
There are several different forms available
on this website as Microsoft Word documents. Please ensure that you download
the correct form below and submit them far in advance of the deadline.
When
prompted to enter network password, click on cancel.

For schools with faculty openings, complete
the Position
Announcement Form (.doc) to order conference listings and announce
a position that is currently vacant or soon to open. Position Announcement
Forms must be received by (not postmarked by)
October 10, 2008 in order to be included in Pre-conference Listings.

For individuals seeking a new position,
complete the Job
Applicant Form (.doc) to order conference listings and have
your information sent to various schools. Job Applicant Forms must
be received by (not postmarked by) October 10, 2008 in order
to be included in Pre-conference Listings.

If you simply want to order copies of listings
without announcing a position or applying for a new position, then download
the Conference
Listings Order Form (.doc).
All of the forms above contain a copy of
the the Credit Card
Form (.doc) if you wish to pay by credit card rather than check.
You may submit the credit card form via e-mail but please follow up with
a signed credit card form send via postal
mail.

Notes about the Forms: |

Within each form above are Placement
Registration and Conference Listing Order Forms for individuals
who wish to order copies of listings of position vacancies and/or job applicants.
Both pre-conference listings and post-conference listings of position vacancies
and job applications are available for order by anyone. Pre-conference
listings consist of all forms received prior to
October 10, 2008. Post-conference listings will include
all forms in the pre-conference listings plus all new listings received
on or after October 11, 2008.

Please use only one form for each applicant
or position.

Use only the forms provided on this web
site. PLEASE DO NOT CHANGE THE FORMATTING OF THESE
FORMS! SMA will not accept modified forms. Just fill
in the blanks as set up being careful that your information is visible
on the screen. Also, do not type beyond one page of information on your
form.

No resumes or vitae will be accepted.
However, these items may be left on a table in the placement area at the
conference for the review by SMA Placement Services patrons.

Forms must be mailed with payment. If you
are unsure if forms will be received by October 10th, you may e-mail the
forms to cdavis2@trinity.edu
but payment must be sent via postal mail.

Please do not FAX forms as they will reproduce
poorly in the listings.

In order to use SMA Academic Placement
Services, you must be a member of SMA in good
standing. To join visit the SMA website for membership
information and membership forms. To utilize SMA Placement Services
at the annual conference, you must be registered for the conference. Please
visit SMA's conference
information page for conference information and registration.
