Job Description Template
To be completed by the hiring manager for vacant faculty, and contract staff positions that need to be posted.

Classified Staff Request
To be completed to recruit/hire permanent or temporary classified staff.

Job Posting Request
This form is only for requesting a staff job to be posted for existing positions in your department (either vacant or being back-filled).

Student Work Authorization
To be completed by Offices and Departments when requesting student work positions.

New Trinity Accounts/Services for non-Trinity Employees Form
The Trinity employee who is authorizing this access can either 1) send this form link to that external person to fill out their required personal information, or 2) fill out this separate form link on behalf of that external person.

Performance Evaluations

Annual Performance Appraisal
To be completed once a year during the Spring for each employee. Duplicate the form for each employee as necessary.

Optional Pre-Evaluation Form
This form is an optional worksheet for each employee. Duplicate the form for each employee as necessary.

Reward and recognition
  • Give Kudos through TSEC
  • Nominate a staff member for a McKinley Award
  • Send a note of appreciation

Resignation/Termination policy
To be completed by professional, clerical or skilled/service/technical employees exiting the university. Does not apply to Faculty or Retirees.

Exit Interview Questionnaire
To be completed by all employees exiting the university. Does not apply to Faculty.

Death of Faculty or Staff
Actions required in the event of the death of active and retired faculty and staff.

Tax Information

U.S. citizens and permanent residents may submit Form W-4, both Federal and State, in order to enable the University to withhold the correct amount of tax from their paycheck.
