What can you do inside the Patient Portal?

  Upload and access health records

Students can upload health related documents for the TU Health Services medical staff to review. Anything uploaded becomes part of your medical record.

  View all upcoming and past appointments

Opt-in to receive text message reminders of appointments at the clinic

  Interact with Trinity Health Service 

View and respond to messages from Trinity Health Services staff securely and privately.

  Access bills for services received

View and download receipts/ bills (also called Walk-Out Statements) for services received after 1 August 2022.

Health Requirements

In our efforts to assist our community in staying well and to provide the most appropriate care to each of our patients, we require all NEW students or students returning from leave (temporary withdrawal) to log in to Admissions Portal and complete the Health Services requirements.

* A student who does not provide the required documentation of this vaccine to Health Services, as required by Texas law, will not be permitted to register or attend classes until the requirement is met.

  • Deadline: July 1

    All incoming (New) students should have completed our health requirements. Instructions are found in the admissions portal. When completed, student will receive an email confirmation of completion.

 

     

    Immunization/Vaccination Requirements

    State Requirements

    The State of Texas requires college students under 22 years old to have the Meningococcal Conjugate Vaccine. This vaccine protects against  four  serogroups A, C, W, and Y. It is referred to as the MCV4 Meningococcal vaccine. This vaccine must be administered within 5 years of the start of classes and at least 10 days before classes start. If you have questions about this requirement, please refer to the “Texas Minimum State Vaccine Requirements…” on the Texas Department of State Health Services website. There is a process for claiming exclusion from the Meningococcal Conjugate Vaccine requirement. That “affidavit” process is outlined on the state website.

    Recommendations

    In accordance with The Centers for Disease Control (CDC) recommendations for college students we recommend (but do not require) you to be up to date on these vaccinations: 

    • MEASLES, MUMPS, RUBELLA (MMR) VACCINES (2)
    • TETANUS, DIPHTHERIA, PERTUSSIS  (Td or Tdap) VACCINE
    • COVID-19 VACCINE
    • VARICELLA VACCINE
    • HEPATITIS B VACCINE
    • POLIO VACCINE
    • HUMAN PAPILLOMAVIRUS (HPV) VACCINE
    • HEPATITIS A VACCINE 
    • SEROGROUP B MENINGOCOCCAL VACCINE
    Immunization Records Release

    The Request for Immunization Records Release form (pdf) can be used to request a copy of your immunization records on file in Health Services. We maintain your student medical records for up to 10 years after your matriculation to Trinity University or from the date of your last visit to Health Services, whichever is later.

    You may obtain your immunization records by submitting the form to healthservices@trinity.edu or faxing to 210-999-8378.

    Personal information

    Data that you provide cannot be viewed by anyone else on the Web and is securely maintained by industry standard SSL (secure socket layer) encryption and decryption technology.

    Data Privacy

    We do not share your information with anyone else without your consent. This health record is separate/independent of your academic records. You as an adult are the only person that has access to your TU Patient Portal. Not 18 years old?  When you reach the age of majority you will have sole access.

    A student authorizes us/Health Services to release information to whoever is named on this form.

    Study Abroad Questionnaire

    All students participating in a study abroad program must first submit a pre-physical exam questionnaire form and then undergo a physical exam. To schedule your physical exam appointment with the Prescribing Provider (MD or Nurse Practitioner).

    Students are required to complete and submit the pre-physical questionnaire to Health Services before their appointment. This form can be completed on the patient portal.
